In order for the surgery to prioritise those patients with the most urgent needs we have changed the way in which patients access appointments.
For all appointments except nurse appointments please complete a klinik online consultation form which can be found here
For children aged 14 and under you will be asked to complete a Healthier Together form. The app can be downloaded here
If you cannot complete the forms for any reason please use option 2 on the telephone and someone will help you complete it.
For nurse appointments only please use option 1 on the telephone
For all administration and other queries please use the online system/ nhs app or choose the correct telephone option when you ring the surgery.
